Housing Specialist  

SUMMARY OF POSITION

The overall responsibility of the Housing Specialist is to coordinate housing and services to Homeless Housing participants to ensure they acquire the skills and resources necessary to become self-sufficient. 

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Specialize in housing program eligibility, implementation and placement;
  • Provide case management to clients
  • Provide current information on community resources including current information on employment, housing, legal options, and other services;
  • Must believe in and act in accordance with the agency’s mission statement and goals;
  • Calculate and deliver Housing Assistance Payments and Utility Assistance Payments;
  • Update client files with Progress Notes monthly;
  • Submit monthly reports on program;
  • Perform monthly home visits and client transportation;

Percentage of time spent doing Coordinated Entry Assessments, providing Supportive Services and housing navigation. Services: Obtain benefits, monitor and evaluate program participant progress; Provide information and referrals to other providers; develop individualized housing and service plan, including a path to permanent housing stability.

 EDUCATION and/or EXPERIENCE

  • Bachelor’s Degree (B.A.) in Social Work or related field.  Knowledge of crisis intervention, case management, social work theory and ethics; 
  • Or, high school graduation, or its equivalent, plus at least two years of experience in social service or human service programs.
  • Or, any equivalent combination of experience and training which provides the required knowledge, skills, and abilities. 

LANGUAGE

  • Ability to read and interpret documents such as operating manuals and professional journals. 
  • Ability to write routine reports, correspondence and appropriate case notes. 
  • Ability to speak effectively before groups of clients.
  • Able to communicate with Spanish-speaking clients or work with an interpreter/translator on program requirements

COMPENSATION

DOE; Occupational & Liability Insurance, Social Security, Annual Sick and Holiday Leave.

HOURS

  • Full-time position, 8:00 AM to 5:00 PM, Monday-Thursday, 8-4 Fridays. 
  • Occasional but very rare evening hours may also be required.  
  • Must have valid Driver’s License

SSVF Healthcare Navigator

SUMMARY OF POSITION

The overall responsibility of the Program Manager is to provide supportive services to Homeless AND Near-Homeless Veterans and their families to ensure they acquire the skills and resources necessary to stably housed.   

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Obtain eligibility documentation, prepare service plan with Veteran and family, assist in completion of personalized service plan, and goals.
  • Provide case management to clients, offering referrals as necessary.
  • Provide current information on community resources including current information on employment and income opportunities, housing, legal options, and other services.
  • Must believe in and act in accordance with the agency’s mission statement and goals.
  • Deliver Housing Assistance Payments and Utility Assistance Payments
  • Update client files with Progress Notes
  • Perform home visits, phone calls and follow-up as necessary  (Must have valid DL)
  • Assist clients with move-ins, including coordinating donations
  • Provide services that include connecting Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care. 
  • Provide case management and care coordination, health education, interdisciplinary collaboration, coordination, and consultation, and administrative duties.
  • Work closely with the Veteran’s primary care provider and members of the Veteran’s assigned interdisciplinary treatment team.

 EDUCATION and/or EXPERIENCE

  • Or, high school graduation, or its equivalent, plus at least two years of experience in social service or human service programs.
  • Or, any equivalent combination of experience and training which provides the required knowledge, skills, and abilities.

  • Ability to use a computer and all operating systems; ability to write routine reports, correspondence and appropriate case notes, use of Excel, Word and Power Point. 
  • Ability to speak effectively before groups of clients and give presentations. 
  • Bilingual preferred.

HOURS

  • Full-time position, 8:00 AM to 5:00 PM, Monday-Thursday, 8-4 Fridays (option to work 9 hour days to get every other Friday off)
  • Occasional but rare evening and weekend hours may also be required.
